Packages and Payment

Payments are due at the time of the first appointment and are non-refundable. Checks payable to "Table to Crave, LLC", cash and Venmo transfers are accepted. Credit card payments may be made online only. Table to Crave does not accept or bill insurance. Pricing includes consultations only. Recommended supplements and testing are charged/sold separately.

All packages must be started within 3 months of purchase date. Packages are valid for the duration of the package.
